Hmm, I watch the show enough that I thought I would recognize it. I did just find this song as a midi on page, but I can't remember it from the show... I just remember the track called Chibiusa on disc 3 of the memorial music box... that one sticks out to me...

If you google "chibi usa's theme piano", you should be able to find sheet music for this song. That's the best I can offer you - I have way too many Sailor Moon soundtracks to hunt down the exact track on the exact cd. But this is her theme.
